Help Me Grow Ventura County Network of Care

Network Profile-Integrator Learning Lab 2020

Through participation in the Nemours Integrator Learning Lab, the HMGVC Network of Care has specifically focused on jointly developing a project with our county’s managed care plan, Gold Coast Health Plan, to leverage Medi-Cal/Medicaid to foster the social and emotional health of Ventura County’s young children (ages 0-5). We continue to develop partnerships with other community agencies and programs to expand the capacity of our Network of Care to support families and children across the developmental spectrum.

*ACEs Aware Grant recipient Landon Pediatric Foundation is a key partner in this network of support.
